two surgeries for type VI MPB
2006 grafts first surgery
1643 grafts 2nd surgery

See the planning to create naturalness in hair line with the use of curvature of hair shaft.
This create a natural looking fronto-temporal angles. the hair flow shows a natural turn of the hairs due to curvature.

The concept behind using the hidden mini grafts if that the patients should get the maximum impact with minimum grafts.

It has to be in such a way that there should be no un-naturalness in the looks.

'Good cosmetic surgery is see by all and recognised by none'.

if my patient gets a remark that wow its a good surgery I think i have failed the job. theuy shouls comment ' hey you lost some weight', 'you looking different, have you changed your hair style' or hey you are thinning out, do some thing'.

dr mohmand,
Excellent result for 3600 fu.

Could this have been done in one pass rather than 2?

yes it could be done. two things his donor area to tight to be mobalised so that i can take more than 2200 grafts.
second some times they cant pay more than that.
but in this particular case his donor area was tight.

Yes i know cost if often a factor. We can all relate to that.
Tight scalp laxity is also an unfortunate condition.

Still, one session or two this patient must be very happy. Good work.

Do you perform larger 3000 fu+ sessions?
Also, what sort of density do u give? I understand theres many different variables that have to be taken into account but where and when appropiate can u dense pack and at what level?

Sorry for the questions but i know you've just been recommended and i'm not overly familiar with your methods and capabilities.

IMO, I would of rather seen a more mature (higher) hairline, allowing dense packing of the grafts in the frontal third; the frontal hairline height IMO is low; with the hairline that low, it makes it harder to create a lot of density, especially when the grafts were placed over such a large area on top; IMO that low of hairling with sparse density is somewhat unnatural; there are some individuals who keep their hairline and diffuse but it is not the general rule; don't get me wrong, the patient definitely has a cosmetic improvement, his face is framed, and it doesn't scream "unnatural", just my thoughts.....

Dr. Mohmand,

You've gotten some impressive results against decent odds..."NW7" or close baldness, and very dark hair (which may be a plus, I don't know...but it seems to me that unnatural placement would be more evident with dark hair).

Dear Josh
my routine sessions are between 2500 to 3500 FUT in one session, I have done up to 5000 in one go but they are far and few in numbe due to the limiting factors of making a minimal visible scar. I am very particular about that. I do not want to mess the back yard to make the front yard beautiful.
I believe in visual density rather than actual density. The fact that God Almighty has provided a depth of 50% a visual density will be achieved with 25 to 35 grafts per sq cm while the actual will be between 45 to 60. My clients have been very happy with density of about 30/sq cm. I am sure some might disagree but most of my clients are more than type V MPB.

Dear Dhoose75

I respect your openion and of so many others for a more mature hair line. Its just so unfortunate for us that our cliental are more demanding and at the nd of the day its the client demand, they are paying for it and I do have to mould my skill to what they want, so what I do is I keep the density low and just play arround with mini grafts and make an illusion and also give them depth.
Mind you not all are as low as these but generally my hair line is placed at 7 cm. I think at 8 cm from glabella the forhead looks rather too big and more bald than i would be happy. In fact if you really see then at 7 cm the baldness is that of type II and at 8 cm it will be like type III.

Dear Bengamin

Dark hair are actually a bit of problem as they stand out on skin and so you can see the skin easily, while the lighter hair can blend well with skin and give a denser illusion. You are very right a bit more un-natural placement and it scream on your face that its so obvious and so un natural.

Dear Balody

In my openion mixng the new technology and old is the winning combination. I can achieve much better results than any other surgeon at a very comparative price for patient.
I think I become much more patient friendly. Fortunately for me as my economics are different from western world and I can afford all this.

Dr. Mohmand,

Very nice photo collage displaying various angles so our members can make an accurate assessment of the results. And a very nice result. This patient clearly has ideal hair characteristics.

To answer Balody's question, I happen to be a fan of all follicular unit hair transplantation without the use of DFUs (double follicular units - aka minigrafts).

However, depending on the hair characteristics, I believe the use of some DFUs can be appropriate for some patients as long as the majority of grafts are follicular unit grafts.

I know a few physicians are convinced that DFUs help aid the illusion of hair density however, given that the number of hairs are the same, I have not been firmly convinced. In my opinion, it comes down more to strategic placement of the grafts more than anything else. So if some DFUs are used, as long as they are strategically placed among follicular unit grafts, I am convinced that the result can come out looking natural.

Very nice results Doctor; and I like your philosophy of "no one should notice my work". You're doing a great job.

Dear Jotronic

I said that not to be arrogant or bad but just to give the analagy that it helps more patients than doctors, in fact its more consumer protection sort of it. having said that I, one must not close the doors for any one, I rise with the creticism. I learn when I am told I am wrong.
"A kite rise against the wind, not with the wind".
so I am open for creticism.

This forum is because of all of you, I just try to bring my patients who are bit more difficult and are advance hair loss.

Hope it will be a bit of clarification on my side.

Hope to hear from you again.

I am glad to see that Dr. Mohmand is doing pretty good work in Pakistan. That area of the world is in great need of quality surgeons. Lets not forget all the patients who cant afford to travel and must have a local dr.

Dr. Mohmand,

Since you do regular larger densely packed sessions of 2500 to 3500 grafts on a regular basis, I encourage you to post some examples of this as well. Immediately postoperative pictures would also be helpful.

It seems like you are doing some really nice work!

Falc
