hey doc

the works looks good and certainly clean , i have a few questions , if you don't mind answering

how many grafts were put into the crown , and how many were used on the temple area ?

also do you sometimes shave the patient down or do you always insert the grafts with the exsiting hair left long ?

alot of guys on here always ask if docs have to shave down , some do , some don't , I'm interested in your take on this

I'd be happy to answer your questions.
This patient had 700 FU grafts placed in the crown area, and 1121 FU grafts
in the bilateral temporal areas.
My staff and I prefer to keep the pre-existing hair long, as opposed to shaving it beforehand. We find that leaving the hair longer makes the creation of the sites and placement of the grafts a bit easier, as the pre-existing hair can be combed from side to side, or forward and backward, etc., giving us greater exposure of the underlying scalp. Also, we feel that, cosmetically, having the pre-existing hair left long places less of a burden on the patient in terms of the procedure being much less conspicuous. Again, I don't beleive there should be any hard fast rule re shaving or not; it really amounts to the individual surgeons preference.

No offense, but Im worried more about the finished product. If shaving is gonna be better for the end result then so be it. Shave it!

This result looks very nice at this stage. I think the density blending into the existing hairs outward into the area where it was more sparse looks natural. Nicely done Dr. Carman.

I don't know why everybody is saying nice result. That is not yet the final results and 1000grafts in the hairline is not going to give a huge impact in terms of density and aesthetics

Thanks doc but grown out pics is what we CRAVE. Ive seen hundreds of immediate post op pics with no final result ever shown. When you have time it would be nice to see maybe 5 postop pics from day one to month 6-10

Anouar I think Pat was the only one who used the word result but was commenting on the work not the final result. You right though 1000 grafts will not be a huge impact . Now if all 1800 were used then thats a different story but maybe they have a second surgery in mind. Some doctors prefer to do it in stages since hardly anyone just needs one surgery. The most impressive hairlines Ive ever seen had two surgeries . There are alot of one and done hairlines but even those could use a little refinement

The session above is for 1821 grafts, not 1000 grafts, and placed in a small area like above should make for a very nice cosmetically pleasing result.

ok Falc. if you want me to be precise I'll do that. 1121 in the front the rest went in the back. You still think he is going to get nice coverage in the front as well as the crown. I seriously doubt it. I had a 1000g in my hairline at the time when my hairline was like his and to be honest it still looked thin

I think you'll be surprised at how a little can sometimes go along way depending on placement and hair characteristics.

If you look at some of the closeups, the hairline is relatively densely packed (as said above approximately 40 FU / cm2).

The average density of hair for those with no loss is 80 FU / cm2. One typically only needs to acheive 50% of their original hair density to achieve an illusion of fullness.

The crown will clearly be a little thinner, but this patient can always go for more surgery in the future. Many surgeons won't transplant too much in the crown on patients with minimal loss and a young age to prevent the possibility of the halo effect. I used the word results, but if you notice I said *at this stage*. After looking at thousands of before during and afters, you get a sense of how things will turn out. Of course we are all interested in the final outcome, I would think many of the other members can tell a good procedure at this stage.
