40 year old class 5v patient desired frontal hair restoration. He was informed pre-op that in one session he could have a very acceptable hairline and reduced crown. A 2 layer tricophytic closure was performed. We were able to obtain 2835 FUT grafts. The patient had excellent donor density. The following number of grafts were utilized. 2215 three and four hair grafts, 315 two hair grafts and 305 single hair grafts. Approx. 9000 hairs were utilized.

To my untrained eye, this patient looks much closer to a NW 6 than he does a NW 5v. This guys number of 3 and 4 hair grafts is crazy. It averages out to about 3.1 hairs per graft because of that. This is a fabulous example to post because it shows that graft count is merely one of a number of factors that go into a fantastic result. Good job doc! Would you say that his hair characteristics were also favorable?

The coarse Asian hair definitely helps with the density but this does appear to be a very pleasing result. Also I don't know what the Dr's charging structure is like but at 3.1 hairs per graft I imagine the patient is more than happy.

Not sure I've ever seen ~3k produce such a dramatic, monumental result -- high hair count, or otherwise.

TC17,imissthebarber and thanatopsis_awry,
Thank you for your positive comments. The patient had amazing density and thick hair. I was very pleased with his results when he returned for his 10 month post-op visit. The patient is very happy with his results.
Regards,
Dr. Keller

LMS, in the write-up he mentioned "2215 three and fours", "315 twos", and "305 singles"....I do wonder just how many were fours as opposed to threes....but either way, like I mentioned before, this could very well be the most magnificent transformation I've ever seen -- pound4pound, or graft4graft. Wink

BTW- his crown was about as bald as they come and LARGE too.. how were you able to close it off so well, throw grafts into the ENTIRE crown area, and still have enough grafts to provide such a dense front half AND hairline with only 2835 grafts?

do you have a graft placement photo of the crown?? that might help me make sense of this a little better....

The crown is Bobman-big, indeed....in fact, if NO hair grown in this guy's ENTIRE hairline and frontal-1/3rd, and JUST to his vertex/crown/midscalp....I think the result would be solid! Insanely magnificent transformation...

This is a great transformation especially given how few grafts were used. He is well positioned to finish off the crown now (if he desires).

That result is mind blowing. If I had to guess by the photos it would have been 3 procedures with about 2,500 grafts in each one.

Hi,
What do you think Asian have less or more hair/cm2 ?
What is approximately density of the donor area in terms of per square cm in this case?

Khizar,
Thank you for your question. Asian hair differs individually. I do not march a stated density from back to front or front to back. I usually match density to be confluent to the sides of the desired hair restoration region. Hair density can be from 38-78 units per cm2. I usually create a strong feathered density in front and the central tuft. Density in these regions vary with skin and hair color. In my opinion the mid-scalp and crown are less important for facial framework and less density, but the larger follicular units are best used here to match naturally occuring follicular units. In the lateral areas of the donor region, where finer hair is found , more hair is utilized for the very front hairline to create stronger natural looking density. This patient did have excellent donor density which allowed him to have optimal results. Thank you everyone for your positive comments!
Sincerely,
Richard S. Keller, MD, ABHRS

Lost my Swagger,
I achieved results as shown by varing the density of grafts. I utilized high density in the front scalp, less density in ithe mid-scalp and larger follicular units in the crown. I will post more photos at the 12 month follow-up visit.
Sincerely,
Dr. Keller

Vincehair,
Thank you for your positive comment. Pricing is around $4.00 per graft with discounts for larger sessions. For repairs and scar revisions the price is slightly higher. There are no further charges from our office, (all medications and post op visits are at no charge).
Regards,
Dr. Keller
