My goal was to get around 4,000 FU for this 45 year old male. He did not have great donor laxity. He had been a boxer and His skin was very tough. I find patients who have played football, boxed, or other contacts sports can have very tough, thick skin. His density was a bit below average and he did not have as many 3 and 4 hair FU's as an average patient.
His session did yield 3,715 FU's but with his size head, thin caliber hair, I still did not plant into most of his crown. He is aware the if he wants his crown done that will be a second session.
The strip removed was as follows
10.5 cm long at 1.5cm width
17.5 cm long at 1.8 cm width
Hair count was:
1 Hair FU 1078
2 Hair FU 2032
3 Hair FU 590
4 Hair FU 15
Total FU 3715
Total Hair 6972

judging by the photos, especially on the sides and back of his head, he looks well on his way to a nw7. dr. shapiro, i take it you think you will have enough donor left to fill the bald spots in should he progress further? and i take it his balding has pretty much been stable for awhile now?

Looks very clean. hdude, the guy is 45, so you'd think his balding would have stabilized by now, right?

Your questions about his future progression are very good ones.

It is hard to know if this patient is stabilized and there is the possibility this patient may progress to a Norwood 7. In a patient like this one who may progress to a Nrowoood 7, I keep the hairline high, do not bring out the temporal peaks and do not go back into the crown but just cover the top of the crown. I also keep the density highest in the central core area and then feather out toward the temporal humps. He has 2,000 to 3,000 more grafts by my estimate and if he looses his temporal humps he still has enough donor hair to fill in and still look natural.
He is 45 years old, has been and has just started on propecia so I am hoping his progression will stabilize and he may even get some growth on his crown. I would not want to do a second transplant on him until we see how his hair loss is progressing.
