doesn't look to diffuse to me, but i am not a physician. Stay on the propecia for a while, if no results, i would see if dr b would play w/ the dose of finasteride for you. 23 is way too young to consider transplant, you still have plenty of hair, stay religious w/ the drug, good luck.

Believe me, we all understand that it is a huge drag to have hair loss.

You still have a decent amount of hair worth saving, and the Propecia route is definitely what I would suggest. Before getting a hair transplant you should (in my opinion) be at least (at least!) a Norwood 3 or up, and have a well-defined bald area (preferrably not the crown). You have the type of pattern where you could really get hurt by shock loss after a surgery.

Good luck, keep your fingers crossed, and hang in there. Hopefully Propecia will do the trick for you.

Are you sure the diagnosis was diffuse pattern alopecia as opposed to diffused unpatterned alopecia? My understanding is that in pattern alopecia there is a definite, well delineated
area(s)of baldness such as what you would see with a receded area of hairline or in round vertex balding. The bald area in these cases would be slick or mostly slick. Most men get this kind of balding, hence the term male PATTERN baldness. Diffused unpatterned alopecia, on the other hand, would exhibit thinning throughout much of the scalp without necessarily having one or more specific areas of slickness. One could have a normally placed hairline but because of the low number of hairs per square centimeter there and in adjacent areas it may look just as unsightly as a fully receded hairline. So a Norwood 2 with diffused unpatterned can still look like shit. Judging by your photographs you appear to have mostly diffused unpatterned alopecia with some pattern balding in the crown, though it's kind of difficult to tell because it looks like you may be doing a combover. Diffused unpatterned does put you more at risk for shock fallout because fu would be placed right in between other hairs. Eventually areas of diffused unpatterned may likely become patterned on the top where DHT does it's thing. In other words, you may end up slick on top. I have diffused unpatterned in the usual donor sites (back of the head and sides)and that makes a successful HT a bitch because there are so few available follicles per cm2 taken from a strip of removed scalp from those sites. I am 52 years old so for me a HT may make sense because my pattern on top is pretty much finalized and I'd be thrilled to have a few more hairs up front and then use some concealer. The issue for me is if I want to permanently scar myself and lose the option of shaving, which I had done for the last 4 years. In your case and at your age your balding pattern has not been finalized. If you start messing with limited donor hair and put it on top, you likely are going to regret it when DHT really takes it's toll in the transplanted area in ensuing years. Take the finastride and see what happens. I have been told that this stuff may in time make the donor area more dense resulting in more available donor hairs. The Woods technique seems to be ideal for men like us because individual follicles can be taken over a large donor area as opposed to a single strip of skin. So I hope docs start using this technique soon. In the meantime for me it's either the scar or no cigar.

I agree w/ Dr bernstien, you may have caught your hairloss early enough to see "some" regrowth, I have read that "regrowth" from Propecia is more possible the younger you are. Remember though, Propecia takes at least 6 months, for me it seemed after 1 year the loss stopped.

Luckily, you have not lost your front hair-line, losing the front hair really can sting.

Stay the course with Dr Bernstien, he has seen a lot of guys like you.
