This man in his late 50s had some very mild thinning in his crown but mostly had a Norwood 3A pattern with a persistent midfrontal forelock. His hair in his midscalp was quite dense.
Many men will keep an island of hair longer than the rest of their hair in the front. Some will keep this island/midfrontal forelock into old age but for most it will eventually disappear. When there is enough room to make a significant improvement, I prefer to transplant right through the forelock. When it is too good to touch, as it was for this man, we elect to transplant around it. He knows that he may have to transplant his forelock later, if and when it thins out.
He had one session of 2658 follicular unit grafts to finish his front, with a mature hairline. He also started Finasteride. He was very happy with the results of his first session but asked if we could lower his hairline further. As his desired hairline did suit his face and it is very unlikely that he will become extremely bald, we transplanted an additional 1791 fug to his hairline, 13 months later, using 21 and 20-gauge needles.
I don’t have a before photo of his crown but he feels that his crown has improved and it does look a bit fuller in other photos. His forelock seems just as thick now as it was 2 years ago. He plans to continue the Finasteride. It may be a very long time before he needs or wants more hair transplantation, if ever. |
