| This is a gentleman in his mid-60’s, who had undergone an extensive face-lift operation 17 years ago. For a time, the scar you see running across his hairline was not noticeable. However as time passed, this patient continued to lose more hair, making the scar evermore prominent. By the time he has come to me, he has virtually no hair in front of the scar. To make his hairline look more natural, we had to bring a new hairline down 1.5 cm below the lowest part of his scar. Our thinking was that if we created enough density in front of the scar, it would completely conceal the scar. There were a few technical limitations with the patient’s surgery. It is difficult to see from these pictures, but most of his tissue within 1 cm either way of his scar has poor blood perfusion, as evaluated based on capillary refill (when you push on the tissue, the redness disappears. Normally when you stop pushing, the redness reappears very quickly. In this tissue, the redness took roughly 2 seconds to reappear). Rather than nice normal scalp tissue, this area had an abundance of tough, non-vascular connective tissue. Due to the nature of this tissue, I decided to proceed with this surgery very concervatively. I was worried that if we put 40 or more grafts per cm into this area at one time, we might experience some degree of tissue necrosis secondary to a lack of blood supply. Therefore, we grafted at approximately 25 follicles per cm, for a total of 1842 units. The plan was to assess the outcome, and proceed with a 2nd procedure of another 25 or more f/u’s per cm 6 months after the 1st surgery. As you can see the photos, his grafts are already coming in quite nicely at the 4 month mark. The patient is very pleased at the progress, and is scheduled for his 2nd procedure in about a month.
